Dr. Leanne Johnston

Clinic Director and Pelvic Floor Specialist 

Leanne Johnston earned her Doctor of Physical Therapy degree from the University of Southern California in 2002. Leanne became interested in treating pelvic floor dysfunction very early in her career. She began taking continuing education classes to learn as much as she could about treating pelvic pain (male and female), pain with intercourse, pelvic organ prolapse, and urinary/fecal incontinence. Helping individuals with pelvic floor dysfunction became her passion. 

Leanne owned and operated an outpatient practice in North Carolina for eight years where she specialized in treating pelvic floor disorders as well as orthopedic injuries. In 2019, she joined Adeo Physical Therapy as the Clinic Director. 

Leanne continues to treat pelvic floor dysfunction and orthopedic conditions, while managing and working with a team of incredibly talented clinicians who provide outstanding care to all their patients. 

 

Moni Kohlhoff

Physical Therapist 

Moni Kohlhoff received her Master’s Degree in Physical Therapy from the University of Colorado Health Sciences Center in 2004 after earning her Bachelor of Science Degree from the University of Wyoming. She joined the team at Adeo Physical Therapy in 2004 and enjoys working with patients of all ages, abilities, and walks of life. Her professional interests include orthopedic injuries, sport related injuries and post-surgical rehabilitation. 

As a physical therapist, Moni focuses on giving her patients the tools they need to live healthier, more fulfilling, and pain-free lives. She has completed continuing education courses in obesity management and nutrition, total joint replacement rehabilitation, shoulder injuries, and foot/ankle injuries to better assist patients in achieving their goals.

Moni was born and raised in Greeley and when she isn’t working, she enjoys camping, fishing, hiking, snowboarding, and playing competitive volleyball.

 

Sylwia Wieczorek

Physical Therapist

Sylwia was born and raised in Poland. She received her Master’s degree in Physical Therapy from the Academy of Physical Education in Katowice and worked for several years as a neurodevelopmental pediatric physical therapist. She moved to Chicago in 2012 in order to advance her professional career and headed west to Greeley in 2019 to join the team at Adeo Physical Therapy as a Physical Therapy Assistant. After much hard work and many hours of studying, Sylwia passed her Physical Therapy licensure exam in June of 2020 and now works for Adeo as a Physical Therapist.

Sylwia’s areas of interest include sports therapy, orthopedic and post-operative therapy, and pediatric aquatic therapy. When not working, Sylwia can be found dancing, hiking, camping, enjoying new cuisines and singing karaoke.
